Do You Have Low Testosterone? What Every Man Should Know

“I think I have low testosterone.”
It’s one of the most common concerns I hear from men in clinic.
Often, they’ve spent weeks or months reading online and watching videos that suggest fatigue, weight gain, reduced motivation, declining athletic performance, or simply getting older are all signs of “low T.” Social media and direct-to-consumer clinics frequently portray testosterone as a universal solution for low energy and declining performance. More recently, the growing public conversation around testosterone therapy has only amplified the belief that many men would benefit from treatment. The reality, however, is considerably more nuanced. Testosterone deficiency is a well-recognized medical condition that can significantly affect health and quality of life. At the same time, many of the symptoms commonly attributed to low testosterone are nonspecific and frequently have entirely different medical explanations. As an endocrinologist, one of my responsibilities is distinguishing true testosterone deficiency from the many other medical conditions that can present in a remarkably similar way. Before discussing treatment, the priority is establishing the correct diagnosis.
Testosterone Is More Than a Reproductive Hormone
Although testosterone is commonly associated with sexual health, its physiologic effects extend well beyond reproduction. Produced primarily by the testes under the regulation of the hypothalamic-pituitary-gonadal axis, testosterone plays an important role in maintaining lean body mass, skeletal strength, erythropoiesis (red blood cell production), sexual function, and normal spermatogenesis. It also contributes to overall physical performance and body composition throughout adult life. Like every hormone, testosterone functions within a carefully regulated endocrine system. Both deficiency and excess can have important clinical consequences, which is why treatment should always be guided by objective evidence rather than symptoms alone.
What Does Low Testosterone Feel Like?
One of the greatest challenges in diagnosing testosterone deficiency is that many of its symptoms overlap with common medical conditions unrelated to testosterone.
Symptoms that are more suggestive of true testosterone deficiency include:
Reduced libido
Fewer or absent spontaneous morning erections
Erectile dysfunction accompanied by diminished sexual desire
Progressive loss of muscle mass or strength despite resistance training
Reduced exercise capacity
Low bone mineral density or fragility fractures
Infertility
While these symptoms increase clinical suspicion, they do not establish the diagnosis on their own.
The Symptoms Most Commonly Misattributed to Low Testosterone
Many men seek evaluation because of fatigue, brain fog, reduced motivation, weight gain, or difficulty concentrating. Although these symptoms deserve careful medical attention, they are not specific to testosterone deficiency.
In clinical practice, these same complaints are frequently caused by conditions such as:
Obstructive sleep apnea
Chronic sleep deprivation
Stress, depression, anxiety or PTSD
Thyroid disorders
Iron deficiency or anemia
Diabetes and other chronic illnesses such as obesity
Medication side effects
Excess alcohol consumption
This is precisely why an endocrine evaluation is valuable. Treating someone with testosterone when the underlying issue is untreated sleep apnea, hypothyroidism, or iron deficiency is unlikely to improve symptoms and may delay the correct diagnosis.
How Is Low Testosterone Diagnosed?
The diagnosis of testosterone deficiency requires both compatible clinical symptoms and consistently low testosterone concentrations on laboratory testing. Neither symptoms nor laboratory values should be interpreted in isolation. Because testosterone secretion follows a normal circadian rhythm, blood testing should generally be performed in the early morning when circulating concentrations are highest. An isolated abnormal value should usually be confirmed with repeat testing before establishing a diagnosis.
Depending on the clinical scenario, additional evaluation may include luteinizing hormone (LH), follicle-stimulating hormone (FSH), prolactin, thyroid function, iron studies, other pituitary hormone testing and possibly imaging of the pituitary gland. These investigations help distinguish primary testicular dysfunction from disorders affecting the pituitary or hypothalamus and may uncover entirely different endocrine conditions requiring treatment.
Why Identifying The Cause Matters
Perhaps the most important part of the evaluation is determining why testosterone is low. Not every man with testosterone deficiency requires immediate testosterone replacement therapy. Testosterone production is highly sensitive to overall health. Anything that places the body under chronic physiologic stress or promotes a persistent inflammatory state can suppress the hypothalamic-pituitary-gonadal axis and lead to lower testosterone levels.
Whenever a reversible cause is identified, the priority is to address that underlying condition. In many cases, optimizing overall health can lead to meaningful improvement, and often normalization, of testosterone levels without immediately committing a patient to long-term testosterone replacement therapy.
Does Aging Mean You Need Testosterone?
No. Although testosterone levels gradually decline with age, this process is highly variable. Many healthy men maintain testosterone concentrations within the normal range well into later adulthood. Treatment decisions should never be based solely on age or on a laboratory value that falls near the lower end of the reference range. Instead, treatment should be individualized and based on the complete clinical picture, including symptoms, biochemical findings, underlying medical conditions, fertility goals, and a careful discussion of the potential benefits and risks.
When Is Testosterone Replacement Appropriate?
For men with confirmed testosterone deficiency, testosterone replacement therapy can improve sexual function, libido, lean body mass, bone density, and overall quality of life. However, testosterone is not an anti-aging therapy, a substitute for healthy lifestyle habits, or a treatment for every cause of fatigue.
The goal of therapy is not to achieve supraphysiologic testosterone levels. Rather, it is to restore hormone concentrations to the normal physiologic range while carefully monitoring for efficacy and potential adverse effects. Successful treatment requires thoughtful dose adjustment, regular laboratory monitoring, and ongoing reassessment rather than a one-size-fits-all approach.
In conclusion, low testosterone is a legitimate medical condition, but it is also one of the most over-diagnosed and oversimplified topics in modern medicine. Not every man with fatigue has low testosterone, and not every man with a borderline laboratory value requires treatment. The best outcomes begin with an accurate diagnosis, a careful search for reversible causes, and an individualized treatment plan grounded in medical evidence rather than marketing.
